Rates
For clients who choose to private pay, my rates for in-person and/or telemental health psychotherapy services are:
- $200 for the intial Intake Assessment
- $150 per 50-minute Individual Therapy Session (after the intial intake assessment)
Missed Appointment & Late Cancellation Policy
Calm Harbors Counseling Cancellation policy requires that the client must notify the provider of their need to cancel at least 24 hours prior to the start of their scheduled appointment time. Cancellations within the 24-hour period leading up to a scheduled session will result in the client being charged a full session fee of $150.00, unless the provider and I agree I was unable to attend the session due to circumstances outside of my control. Please note that late cancellation or a “no show” fees are not covered by health insurance and will be charged to the client’s credit card on file.
Insurance
Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for psychotherapy services.It is recommended that you consider asking these questions to your insurance provider to help determine your benefits:
- Does my health insurance plan include mental health benefits?
- Do I have a deductible? If so, what is it and have I met it yet?
- Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
- Do I need written approval from my primary care physician in order for services to be covered?
Calm Harbors Counseling currently accepts the following health insurance plans:
- Blue Cross Blue Shield (BCBS) HMO Blue Plans
- Point32 Health (Tufts & Harvard Pilgrim HMO/Commercial Plans)
- Medicare
Out-of-Network Benefits
Clients who do not health insurance coverage through the aforemention manage care entities are well to self-pay and submit claims to their insurance company to be reimbursed. Calm Harbors Counseling is happy to provide client’s proof of payment for sessions, in the form of a SuperBill that the client can than submit to their insurance company for reimbursement. Please note, that that submitting to an insurance company for reimbursement of out-of-network benefits is the responsibility of the client and that Calm Harbors Counseling will not be involved in this process. Any out-of-network claims denied by an insurance company are the finanicial responsibility of the client.
Payment
Payment is due at the time of each session, unless otherwise agreed upon between the provider and client or insurance coverage that requires another agreement. Calm Harbors Counseling requires all clients to complete a Credit Card Pre-authorization Form giving CHC permission to obtain payment of fees for services rendered from credit card account indicated on the Credit Card Preauthorization Form.
Good Faith Estimate
Calm Harbors Counseling will make every effort to best estimate the cost of services accurately before appointments to minimize the need for modifications later. However, at times, service codes change and result in either additional or decreased fees dependent upon your individual health insurance plan’s deductible or co-insurance benefits. These modifications will be rectified accordingly. As a result, the exact amount cannot be guaranteed in advance of your visit (See “No Surprises Act Notification” Form).
